Health quiz information

The Health Evaluation button opens an external Youngevity product quiz. Answers are entered on that external website. This coaching website does not collect those answers or automatically receive quiz results. Review the quiz’s privacy information before submitting personal or health information.

Consultation form

The Before Your Consultation form asks for your name and email and optionally your contact details, health history, medications, supplements, goals, nutrition, mental health, lifestyle, and spiritual health. Sheena uses the information you choose to share to prepare for your consultation. Sending the form does not schedule an appointment.

Email delivery and consent

When you submit the consultation form, your answers are sent to FormSubmit for email delivery to Sheena at sheena@highertransformationcoaching.com. The form prepares a printable PDF in your browser to attach to the submission email. If PDF preparation is unavailable, your answers can still be sent in the email. The form asks for your consent before sending. FormSubmit and Sheena’s email provider process the submitted information. The consultation form does not send your answers to the Youngevity quiz or store a copy in this website’s database.

Storage and retention

Draft answers stay in the form while the page is open and are not deliberately saved in browser storage by this website. FormSubmit states that it retains submitted forms for 30 days. Submitted forms also remain in Sheena’s email inbox until deleted; a specific inbox retention period has not been established here. Contact Sheena to request correction or deletion. If you choose to print or save a copy, you control that copy.

External shopping link

The Shop Youngevity button takes you to a shopping website hosted by Youngevity. Youngevity handles information under its own privacy policy. Purchases and checkout are not processed by this coaching website.

Website analytics

With your permission, this website uses Google Analytics to measure page visits, navigation and button clicks, scrolling, downloads, and consultation-form activity. Google receives device and browser information, approximate location, referral information, and pseudonymous cookie identifiers. The analytics integration does not send your form answers, name, contact details, health information, or PDF attachment to Google. Form reports count starts, section numbers, send attempts, and returns to the confirmation page; they do not prove that an email was delivered. Advertising personalization is disabled.

Choose Allow analytics or No thanks in the cookie notice. You can change your choice using Cookie settings in the footer. A browser Global Privacy Control signal keeps analytics off. Your choice is stored in this browser, and a temporary anonymous submission flag is used to avoid counting confirmation-page reloads as new submissions. No health answers are stored in that flag.

Google processes analytics information under its privacy policy at policies.google.com/privacy. These reports do not identify individual visitors by name.
